Congenital Subcutaneous Angiolipoma of the Upper Back in a 4-Year-Old Girl: A Case Report

Abstract

Introduction: Angiolipoma is a rare benign mesenchymal tumor characterized by mature adipose tissue admixed with proliferating vascular channels. Although commonly reported in young adults, it is uncommon in the pediatric age group and may clinically mimic other benign soft-tissue lesions such as lipoma, making diagnosis challenging. Case Presentation: A 4-year-old girl presented with a painless swelling over the upper back that had been present since birth. Clinical examination suggested a lipoma. Magnetic resonance imaging (MRI) revealed prominent vascular channels within the lesion, raising suspicion of a vascular component. The lesion was surgically excised, and histopathological examination demonstrated well circumscribed non encapsulated mass on gross, on microscopy mature adipose tissue interspersed with numerous vascular channels, confirming the diagnosis of angiolipoma.

Conclusion: This case highlights the importance of thorough radiological assessment and histopathological confirmation in the evaluation of atypical pediatric soft-tissue swellings. Recognition of vascular features on imaging can aid in preoperative planning and help differentiate angiolipoma from other benign soft-tissue tumors.
